I am writing on behalf of the Office of Consumer Affairs and Business Regulation, as the oversight agency for the HIC Act, M.G.L. Chapter 142A, to provide you with an additional resource to protect your resident homeowners who have entered into a contract with a contractor for home improvement renovations or work on their primary home.
We hope that you will find the attached “Affidavit” a useful document to use as a Supplement to a Permit Application from the Contractor, in Home Improvement Contracting work.
During the course of a recent complaint hearing against a contractor, the municipal building official submitted a version of this document that his town utilizes to obtain additional information from the applicant contractor.
The Form ensured further protection for the homeowner and the town should be applauded for taking the initiative to require the supplement for permit applicants.
